Ordering A Vital Record
State law limits access to vital records less than 100 years old to individuals named on the record, their spouse, registered domestic partner, descendants, parents or guardians, or their designated attorney, federal/state/local government, or genealogist or agent. You will be required to provide your name, address, date of birth, and driver license number for electronic identify verification, and appropriate proof of relationship documents when placing an order ($1.85 fee per transaction). The identity verification fee is non-refundable and applies whether or not the municipality is able to fulfill your order.

As part of identity verification, we use LexisNexis to securely verify your identity. Please ensure all personal information you provide is accurate and up to date.
In some cases, identity verification may fail due to:
- Incomplete or inaccurate information entered
- Limited or outdated public records
- Mismatches between your data and trusted sources
- Recent changes to your personal details (e.g., address or legal name)
If your identity cannot be verified automatically, a manual verification process will be required, which may delay the processing of your order.
Arkansas Vital Records is required to verify proof of relationship and eligibility.
Each customer must provide documentation that demonstrates the relationship between the customer and the name on the document they are requesting.
Please attach documents such as: birth certificates, marriage licenses, wills, insurance, court orders, letters of authorization etc. linking you to the certificate holder or authorizing you to receive the record.
